MEETINGS AND EVENTS REQUEST FORM

DIRECTIONS: Please provide the following information to complete the church use request for your meeting or event. Note that all requests for church use must be approved by Pastor Jefferson prior to the meeting or event date. All requests need to be submitted 7 days in advance of the event date, but we highly encourage early submittals as dates are on a first-come, first-serve basis. Upon submission, your request will be reviewed and submitted to Pastor Jefferson for approval and coordination.
