REQUEST FOR CHURCH TRANSPORTATION

DIRECTIONS: Please provide the following information to complete your transportation request. Note that all requests for church-provided transportation must be approved by Pastor Jefferson prior to the date transportation is required. All requests need to be submitted 7 days in advance of the travel date. Upon submission, your request will be reviewed and submitted to Pastor Jefferson for approval and coordination with the transportation ministry.
