TABLES AND CHAIRS REQUEST FORM FOR CHURCH MEMBERS

DIRECTIONS: Please provide the following information to complete your church tables and chairs request for your meeting or event. All requests need to be submitted 7 days in advance of the event date, but we highly encourage early submittals as dates are on a first-come, first-serve basis. Borrower is responsible for returning all furniture in good condition. Upon submission, your request will be reviewed and submitted to the Trustee Board Church Committee for approval and coordination.

• Nineteen 6' rectangular folding tables

• Forty 5' diameter round folding tables

• Eighty-five folding metal chairs
